A certificate of temporary incapacity (Form 037/u) is a medical document confirming temporary exemption from classes at an educational institution due to health reasons. Issued to school pupils, college and university students, as well as children who have undergone treatment or medical observation.

The certificate is issued for:

• acute respiratory conditions
• infectious diseases
• exacerbation of chronic conditions
• temporary deterioration of health preventing attendance

• date of physician visit
