The Medical Advisory Commission (MAC) Report (Form 026/u) is an official medical document issued following a comprehensive review of a patient's health by the Medical Advisory Commission of a healthcare facility. The report contains the Commission's collective medical opinion and recommendations regarding education, fitness for work, the need for treatment, or ongoing medical supervision.

When is a Medical Advisory Commission Report Required?

A MAC Report may be required for:

• Submission to educational institutions (kindergarten, school, or college)
• Documentation of medical indications or restrictions related to education or attendance at educational institutions
• Confirmation of a patient's health status and the need for ongoing medical supervision
• Submission to government agencies or other organizations

What Does the Medical Advisory Commission Assessment Include?

To prepare the report, the Commission reviews the patient's medical condition and medical records, including:

• Findings from consultations with specialist physicians
• Results of laboratory and diagnostic investigations
• Information from the patient's outpatient medical record or medical history
• Medical indications and contraindications

Following the Commission meeting, an official report is issued outlining the diagnosis, medical recommendations, and the Commission's decision.
