Sanatorium-Resort Card (Form 069/u)

Sanatorium-resort card (Form 069/u) is a medical document prepared before referring a patient for sanatorium-resort treatment. Contains detailed information about the patient's health condition, diagnosis, examination results, and treatment recommendations, enabling sanatorium physicians to select the optimal health improvement and rehabilitation program.

Form 069/u is prepared:

• before traveling to a sanatorium
• before sanatorium-resort treatment
• for rehabilitation and health improvement programs

To obtain Form 069/u, a comprehensive examination is conducted:

• therapist or GP examination
• electrocardiography (ECG)
• fluorography
• laboratory tests (complete blood count and urinalysis)
• additional examinations as indicated
